MAC Tool - Tiered Pricing for Service Levels

Customers have various margin tiers for customers. Add ability to set margin by tier. Allow user to define tier name and add up to 5 tiers (for now).

Info Panel needs to have a reference to this Tier called "Service Level". Service Level should be a custom field for the Admin user to create themselves.

Examples of Tiers could be "Deferred", "Standard", "Premium".
