Update Ground Template with UNLOC

Add CY City UNLOC column and CY Via City UNLOC

Update CY City Comment box title to "CY City (REQUIRED) This indicates the City in which the CY can be found. Valid Values are City, Port Names, Zip Codes, sequences of Zip Codes or zone definition ID's".

Update CY State Comment box title to "CY State (OPTIONAL) This indicates the State in which the CY can be found. Valid Values are 2 character state codes - only fill this out for US Locations".

Add a Column between CY City & CY State to be "CY UNLOC". The Comment box would show "CY UNLOC (OPTIONAL) This is the UNLOC associated with the CY. Only the UNLOC is an acceptable value".
