Single charge code with simultaneous cost + revenue entry

Allow a single item/service (charge code) to calculate both cost and revenue directly in one entry, going beyond today's Resale Item mechanism — which creates a separate, automatically-linked resale item for the paired expense/income side — so managing a charge that pairs a cost with its revenue doesn't require two linked records.
